Some games don't work/lag using speed hack slow motion. Why?

I'm guessing it has to do with vsync/frame rate settings within the game? This game I am playing right now from 2006 lets me use cheat engine, however, when I enable slow motion speed of 0.250 for example, the game will be laggy/stuttery, not smooth slow motion. This game has absolutely not graphics settings. Nothing. Is it just that some games cannot work using slow motion? Faster speeds work fine of course but I'm only interested in slow motion (I mainly use it for recording purposes, not online or for cheats)
