you can get a handle to each of your controls in your dialogproc when handling WM_INITDIALOG then use setwindowtext to change the static text control's contents.
at work right now, will upload when i get home

| I think it's best for you to start at the basics of C++ first, before you start anything like this. Help doesn't come falling from the sky, you know. There will be no tutorials on this, you need to figure it out yourself, and you need programming knowledge for that.

| You are trying to make a DLL that is supposed to be injected into a process, am I correct? Try learning about DLL structures first. And making windows.

yes you can use that as well. anyway here is my example :
http://mn.zwit.org
it shows all of the things you wanted. i didn't get round to filling in some of the boxes.. didn't really know what to put in them. i coded that some time ago as a proof of concept sort thing for myself..
picture of the program :

it's neither. it's assembly language but i'm using macros. saves thinking up label names..
not sure how the include file is done for C++ but i expect it ain't too different
anyway this is the steps i used to make the program :
1 ) design the GUI in a resource editor
2 ) create a dialogproc which handles messages sent to the dialog. have cases for different messages then different cases within each message case for each different handle that is involved
3 ) call dialogboxparam to create a modal window
maybe you should learn c++ properly and then learn basic winapi properly before advancing to window programming. i won't say it's 'advanced' but you'll just end up a copy and paste coder like void/: if you continue on this path without establishing a good base
